Welcome to the coaches evaluation form. Your participation in this evaluation will help DAGSA learn where we can provide improvement in our coaches to ultimately make the playing experience for your daughters as pleasant as possible. To do an evaluation, please select the coach you wish to evaluate from coaches drop down. Once a selection is made the girls that play on that coaches team will be populated in the players drop down. If a player does not show it means that an evaluation has previously been done on her behalf. Once a player has been selected, the questions will be displayed. Please answer all questions. A comment area is provided with each question if you wish to provide addtional information. All evaluation results will be kept confidential and will be used only between the DAGSA president and the coach. Thank you for participating in the coaches evaluation process.
